PainCoach app for controlling postoperative pain at home after knee replacement: effective?

Since knee replacement surgery is a large operation with extensive tissue damage, postoperative pain can be expected. However, postoperative pain can be excessive and limiting the rehabilitation. This is of course undesirable. During the patients’ hospital stay their pain is usually under control since professional help is at hand for providing timely and correct pain management medication. In their discharge consultation patients receive instructions about postoperative pain and which medication they need to take to control their pain at home for the first two weeks. However, limited evidence is available about patients’ pain perception in these first two weeks. Experienced in our practice is – although the instructions given – controlling their pain was more complicated than expected. Therefore, we developed the PainCoach app. This app gives patients advices on medication use, physiotherapy exercise, ect. suited for their pain level in the first two postoperative weeks. In a randomized controlled trail we investigated if this PainCoach app helps our patients in controlling their pain at home.

In your internship you will join us in our daily work. You will create the database, analyse the data and write a scientific paper of above mention study. An extra task during your internship will be collecting patient reported outcome data (PROMs). You will be under supervision of our research coordinator and one of our science experienced orthopaedic surgeons. To understand your research project we would like to give you the opportunity to watch our orthopaedic practice, such as knee replacement surgeries on the surgery complex and pain management on the nursing ward. Of course, we would like to know from you what you want to learn.

Which factors predict a good result after total knee replacement?

‘Doctor, what is my chance on a good result after a surgery?’ That is the main question asked by patients during their consultation with orthopedic surgeons. Our patient groups results are online available and several studies described predicting factors for a good outcome. However, we would like to improve our answer by making it more patient specific and give each patients his individual advice; specifically for our osteoarthritis patients on the decision to undergo a total knee replacement or not. We suggest that the answer on this question depends on the patient’s characteristics, patient’s preoperative function, quality of life and pain and patient’s goal. We would like to investigate which factors are predictive for a good result after a total knee replacement.
